Application should be submitted by email, fax, or mail to Consulate-General of Japan in Denver (see attached for list of consulate offices) by September 26th, 2014. A duplicate copy should also be sent to the U.S.-Japan Council, 2207 Colby Avenue, Los Angeles, CA 90064. Please note "Application for the Japanese American Leadership Delegation" on the front of the envelope if you mail the application.
