Quarantine Measure for Foreign Nationals Who Hold a Japanese Visa

Please read all information carefully. Also, please be aware that not all visas are permitted to enter Japan at this time and the information below only applies to visas that are currently allowed entry into Japan. For information regarding permitted visas, please contact the Visa Section at 303-534-1151.

Upon entering Japan, a negative COVID-19 test result conducted within 72 hours of departure is required to enter Japan.
 
  • Certificate of Testing for COVID-19 [WORD 31KB] that is filled out and signed by a medical institution is considered a valid test result, so please use this form. 
  • If a medical institution does not accept the above form, a different format containing all of the required information listed below in English is acceptable, however, omitting any information may result in denial of permission to enter Japan in accordance with the Immigration Control and Refugee Recognition Act.
          (1) Personal Information of Visa Holder: name, passport number, nationality, date of birth, and sex
          (2) Proof of Authenticity of Test Result: date of issuance of certificate, test method (limited to those listed in form), test result, date of test result, and date and time of sampling
          (3) Medical Institution's Information: name, address, doctor's signature and/or doctor's stamp seal

Visa holders are required to show the original or a copy of the negative COVID-19 test result to the Quarantine Officer upon arrival to Japan. Afterwards, please submit the test result to the Immigration Officer. If the visa holder cannot submit the test result to the Immigration Officer, they will be denied entry into Japan based on the Ordinance for Enforcement of the Immigration Control and Refugee Recognition Act.

If the visa holder enters Japan with false documents, their status of reseidence will be revoked and will be subject to deportation based on the Ordinance for Enforcement of the Immigration Control and Refugee Recognition Act.

*If the visa holder is required to show the test result to airline staff before departure, please adhere to the airline's policy and follow given intructions.
**If a PCR test for an asymptomatic person is not available in the visa holder's country/region of residence, please be aware that they will need to stop in a third country where testing is available before departing for Japan.
